Start with everyday routines
Ask which activities the person wants to manage independently and where assistance would be welcome. Discuss meals, companionship, personal routines and the support already provided by family or friends. Include the person receiving care in decisions wherever possible.
Prepare questions for the care team
Discuss the proposed tasks, caregiver experience, visit availability, communication arrangements and current costs. Agree who the main family contact will be and how changes in needs should be reported. Medical treatment and skilled healthcare services should be discussed with the appropriate licensed professionals.
